Transitional Living Program

Due to COVID-19 Precautions & Mandates:

Our Youth Transitional Living Program (TLP) is open and operating as usual, taking all necessary precautions to safeguard residents and staff.

The Transitional Living Program (TLP) serves 8 youth at a time–up to 20 youth per year–in supervised apartments for homeless youth, ages 16 to 21, to help them make a successful transition to life on their own. The TLP program also serves pregnant and parenting teens and their dependent children.

Residents of TLP work closely with a case manager to establish and achieve goals.  Staff assists youth in: accessing needed community services such as medical and mental health care; learning life skills; accessing education and employment services; and identifying supportive adults and mediating conflicts.

Youth and community members interested in learning more about the Transitional Living Program are invited to attend a group orientation session.  These sessions are held every Wednesday at 3:30pm at 59 Dongan St., Albany, NY 12202.  For an application, youth can contact the Intake Case Manager at 518 689-2129.

Project Independence provides follow-up services to TLP graduates to support their reintegration into the community and to homeless youth in need of housing support. Project Independence provides participants with weekly case management, monthly peer support, independent living skills groups, and a rental subsidy for up to two years.
