Health Home
Equinox is a member of a Health Home called Capital Region Health Connections – a network of health and community agencies working together to help people with serious and chronic health issues get the services they need to live healthier and safer lives.
Who is Eligible?
- Medicaid-eligible individuals
- One or more chronic conditions: such as, mental illness, HIV/AIDS, diabetes, substance abuse, or obesity.
- An individual must be assessed and found to have significant behavioral, medical, or social risk factors that deem them appropriate for Health Homes services.
How Do I Access Services?
Each person who joins is matched with a care manager who coordinates services – from medical care to social and housing needs
The Health Home approach was developed as part of the federal Affordable Care Act reforms to better address complex health needs, to reduce the frequency of unnecessary and costly hospitalizations, and to reduce health care costs. Capital Region Health Connections is a program of St. Peters Health Partners.
