Supportive Case Management
Supportive Case Management (SCM) staff helps clients live as safely, productively, and independently as possible. The program is based in our offices at 845 Central Ave., but case managers make regular home visits to help clients build skills in budgeting, household management, using public transportation, and problem-solving. Case managers also give clients support and guidance about setting and meeting employment or educational goals, and will advocate for clients if they become involved in the legal system.
Intensive Case Management
The Intensive Case Management (ICM) program helps link clients with serious mental illness and multiple needs to community resources, such as psychiatric, addiction, medical, and dental services as well as social, financial, housing, educational, and vocational services. ICM services are delivered in the community and have a low staff to client ratio. ICM was created to help reduce the number of hospitalizations, crisis contacts, and incarcerations.
